The PRP is a tool to help see where God has worked renewal in your life and to plan for further personal renewal. The PRP simply helps us to see ourselves more clearly - where we have grown and where we can grow.
The PRP is not a test. As a matter of fact, you probably shouldn't get a perfect score. It's simply a tool to help guide you to a deeper, richer relationship with our Creator and Savior.
Directly below, you will find two versions of the PRP: a fillable version that allows you to fill out and save the PRP on your computer and a print version that you can print and fill out by hand.

For by the grace given to me I say to everyone among you not to think of himself more highly the he ought to think, but to think with sober judgement, each according to the measure of faith that God has assigned. Romans 12:3 (ESV)
